    Want to improve safety? Start by understanding people — not just procedures. In high-risk work environments, we often focus on systems, compliance, and controls. But it’s human factors that often decide whether things go right… or terribly wrong. Human factors in safety isn’t about blaming the person — it’s about understanding the person: What are they seeing, hearing, and feeling? Are they fatigued, rushed, or under pressure? Is the task designed for success, or primed for error? When we design work with human performance in mind, we move from: ❌ “Who made the mistake?” ✅ To: “What conditions set the stage for it?” Human factors means: Clear, intuitive procedures Fit-for-purpose tools and environments Mental workload and stress considered in planning Control room and field tasks aligned with real-world use Teams trained in decision-making under pressure Because safety isn’t just technical — it’s human. If we want fewer incidents, we need to understand the people doing the work.     Human error is not the cause… it’s the consequence. We often rush to blame people after incidents: “Why didn’t he follow the procedure?” “Why did she ignore the rule?” But modern safety science tells a different story: When unsafe behavior is repeated, the system "not the person" is usually at fault. Think of a work system that assumes: • The worker never gets tired • Never gets distracted • Always reads instructions • Always makes rational decisions That’s not a system, that’s a fantasy. In the real world? Fatigue, pressure, uncertainty, and repetition are always in play. Poorly designed systems create human error. Well-designed systems reduce the chances of it. Today’s safety thinking embraces the principle of “Designing for Human Error” building procedures and controls that: • Align with human limitations • Reduce complexity • Detect mistakes before they escalate Here’s the truth: Don’t overload the worker.     In Human Error (1990), the late James Reason offers a deep dive into the psychology of human mistakes. Over three decades later, his insights remain relevant, whether you're dealing with medical errors, AI design, or aviation safety. Here are a few lessons I took away: Errors happen on three levels 1. Skill-based (slips/lapses); routine actions gone wrong; 2. Rule-based; applying the wrong rule to a familiar situation; 3. Knowledge-based; flawed reasoning in unfamiliar territory. Cognitive underspecification When situations are unclear, our brain defaults to what seems to fit; “I did it automatically.” These automatic processes, similarity-matching and frequency-gambling, are incredibly efficient but also prone to failure. The ‘fallible machine’ If you build a thinking machine that works like the human brain, it will also make human-like mistakes. Reason’s simulations show that our brain relies on smartly organized, but occasionally error-prone, knowledge networks. Error detection: easier said than done We quickly correct low-level errors, like posture or speech. But at higher levels, like planning and reasoning, errors are much harder to spot. Timely and high-quality feedback is important. Disasters rarely stem from a single error The greatest risks come from latent errors in systems; poor design, weak training, flawed decision-making. For instance, Chernobyl, Bhopal, or Challenger weren’t single catastrophic mistakes, but accumulations of small, hidden failures. How do we reduce risk? - Error-tolerant design; - Intelligent decision support; - Awareness of error traps in systems and workflows. Reason ends on a sobering note, when he writes that not all risks can be eliminated, especially those emerging from group dynamics and organizational structures.     ✈️ Human Factors don’t fail. People do — under pressure. This is one of the core ideas I develop in my book on Human Factors in Aviation, built on years of real operational experience, not classroom theory. In aviation, most events don’t happen because of lack of knowledge. They happen because of how humans operate in real conditions. 🧠 Fatigue that is “manageable” ⏱️ Time pressure that becomes normal 🗣️ A doubt that remains unspoken 📋 A procedure followed without thinking 💬 A communication assumed, not confirmed Nothing breaks suddenly. Safety erodes one decision at a time. Human Factors are not about blaming people. They are about understanding how good professionals make poor decisions when context, workload, stress, and expectations align. Talking about Human Factors is not academic. It’s operational.     Human Factors: The Real Safety System in the Cockpit Modern aircraft are incredibly advanced — but human performance still defines safety. For pilots, understanding Aviation Human Factors isn’t academic. It’s operational. Key components every pilot should actively manage: 🔹 The Pilot (Human Performance) Fatigue, stress, situational awareness, workload, and decision-making. Knowing your limits is as important as knowing the aircraft. 🔹 Aircraft & Automation Mode awareness, automation management, and maintaining manual flying proficiency. Always know what the aircraft is doing — and why. 🔹 Operating Environment Weather, terrain, airspace complexity, time pressure, and operational demands all shape pilot performance. 🔹 Procedures & SOPs Checklists, standard callouts, and task sharing exist to reduce error — especially when workload is high. 🔹 Crew Resource Management (CRM) Clear communication, assertiveness, leadership, and managing authority gradients. Good CRM turns a crew into a safety net. 🔹 Threat & Error Management (TEM) Anticipating threats, detecting errors early, and recovering before they escalate into unsafe situations.     ⏳ Late Human Factors = months/years lost + millions spent 💵 Over the next few weeks, I’ll be sharing personal, real-world examples of why integrating #HumanFactors earlier leads to: - more intuitive, safe, and effective devices - meaningful time and cost savings - successful regulatory submissions Over the last decade, I’ve seen many companies only consider HF at the V&V stage 💔 1️⃣ A pre-summative study reveal the need for an entirely new training programme 2️⃣ A validation fail because the user interface didn’t adequately consider colour-blind users 3️⃣ A validation study uncover a mechanical failure that should have been caught during verification 4️⃣ An FDA pre-sub meeting confirm that simulated use alone wasn’t sufficient. Actual-use testing was required 5️⃣ Two rapid formative studies + ongoing expert HF reviews (over five months) cut six months off the development timeline (earlier validation confidence, faster submission, investor timelines met) 6️⃣A device that was intuitive by design become less usable once the IFU was used All examples will be shared in an anonymised way, with full respect for client and device confidentiality.     To me, UX is nothing but the psychology of people interacting with their environment, including products and services, studied across multiple levels, from individuals to groups. UX is not a toolset, role title, or a checklist of methods. It is a way of understanding human behavior in designed systems, unfolding over time and shaped by context, constraints, and social dynamics. That is why learning UX is not about mastering Figma, running a few usability tests, or memorizing heuristics. Those are execution skills. The foundation lives elsewhere. I believe, if you want to truly learn UX, these are the fields you need to study: 1️⃣ Cognitive psychology. This is the backbone of UX. Perception, attention, memory, mental models, decision-making, learning, and cognitive load explain why users behave the way they do and why many designs fail even when they look clean.     Ever wonder what “Human Factors” is? I often get questions about it (my PhD is in Human Factors). In a nutshell, HF is the science behind designing systems – like websites, apps, or even airplane cockpits – to perfectly fit the way humans interact with them. It's a broad field that's been around for decades, and it's had a major influence on UX research as we know it today. Human factors, human-computer interaction, and user experience are three closely related fields. Here’s a quick breakdown: • HF: Established during WWII, it focuses on human interaction with various systems, not just computers. This includes car dashboards, medical devices, and more. • HCI: A subfield of HF that emerged in the 1980s with the rise of personal computers. Its focus is more academic, specifically on designing and evaluating how we use computer systems. • UX: The most recent of the three (coined in the 1990s), UX centers on creating smooth and enjoyable experiences for users interacting with digital interfaces.     ✈️ New Publication Alert! ✈️ Excited to share my latest work, “Enhancing Flight Deck Resilience and Optimizing Risk Mitigation: A Sociotechnical Approach,” now published in Human Factors in Design, Engineering, and Computing as part of the AHFE 2024 International Conference proceedings. This paper explores the flight deck as a sociotechnical system, emphasizing the critical role of socio-processing capacity—the cognitive, affective, communicative, and collaborative abilities pilots use to manage information, coordinate with their crew, and make informed decisions. One key finding: when the flight deck environment lacks psychological safety, pilots are more likely to self-silence, shifting away from open communication. This directly impacts the effectiveness of safety models like Threat and Error Management (TEM). The research highlights how fostering psychological safety and enhancing interpersonal skills can strengthen collaboration, bolster resilience, and ultimately reduce risks in the flight deck.
