Antarctica is one of the most remote and demanding environments on Earth, where extreme conditions and limited medical resources make thorough preparation essential. To help ensure the safety of all participants, a comprehensive medical and dental evaluation is required for deployment. Each participant will receive a Physical Qualification (PQ) Packet with the forms and instructions needed to complete these requirements. While the PQ process involves a significant amount of paperwork and requires careful attention, it is a vital step in preparing you for the remarkable adventure ahead.

The Antarctic Support Contractor (ASC) processes all USAP participant deployment paperwork.

USAP medical and travel qualification forms are only valid if completed and submitted by individuals who have received a formal offer of employment and/or official U.S. National Science Foundation approval for deployment to Antarctica. Forms submitted without a valid offer or approval will not be processed for employment or deployment.

NOTE: ASC no longer mails deployment packets; they must be completed electronically and printed by the participant.

  1. Complete the PQ Packet emailed to you by UTMB. For detailed instructions and additional information on physical qualification, refer to the Medical Physical Qualification (PQ) page.
  2. All participants can access and print travel forms at the following location: Travel Deployment Packet
Submit all completed travel forms no later than eight weeks before your deployment to the following address:
